(A) Approved CTF Participants as of April 21, 2016
Step1: Check to see if your entity is on the list of organizations located in unserved and underserved areas. If your organization is on the list, proceed to Steps 2 and 3.
If your entity is not on unserved and underserved list and your entity can only access Internet services through dial-up phone service, you may request an informal appeal with CD by:
Submitting a letter to CD, signed under penalty of perjury by a superintendent, chancellor, executive director, chief executive officer, or other executive officer of comparable position of your entity, certifying that your entity has contacted all carriers in the area, and that no carrier is able to provide broadband service. The letter shall also include a list of the carriers in the participant’s area.
Proceed to Steps 2 and 3 below.
Step 2: If your entity is on the list, you should receive a CPUC email or letter requesting your entity to fill out and return the CTF Self-Certification Form for Exemption from Reduced Voice Services Support with copies of billing documentation, your most recent bill and a bill from the previous year.
Step 3: No later than June 14, 2016, send the Self- Certification form and required billing documentation to: CTFExemptions@cpuc.ca.gov.
Once the Communications Division (CD) staff has completed the review of your application for a Voice Exemption, you will be notified by e-mail of your application status. List of participants approved for Voice Exemption will be posted on the CTF website
(B) New Applicants, applying to the CTF program after 4/21/2016
Please complete the CTF Application including the Voice Exemption Addendum. If your organization is approved for the Voice exemption, you must provide the approval documentation to your service provider.
