Download decision D-97-06-107 and a copy of the registration form in the desired file format. When the Registration form has been completed, the original form plus seven copies must be filed with the Docket Office of the Commission. A registration fee of $75, check, draft or money order payable to CPUC, must be enclosed with the registration forms.
