General Order 133-E Annual Fine Tabulations
When POTS and fixed interconnected VoIP carriers issue automatic customer credits or incur fines during a calendar year for non-compliance with GO 133-E, these providers are required to provide an annual report as a Tier 2 Advice Letter using the Annual Fine Tabulation template. The Tier 2 Advice Letter must be submitted within 60 days after the end of the calendar year.
The annual report must include the following for the specified reporting year:
1. Automatic customer credit amount issued due to Installation Standard and POTS and VoIP Outage Repair Standard noncompliance
2. General Fund fines resulting from violations of the POTS and VoIP Outage Repair Standard
3. General Fund fines resulting from violations of the Customer Service Standard

Annual filing schedule
- The annual fine tabulation must be submitted as a Tier 2 Advice Letter to TD._PAL@cpuc.ca.gov within 60 days after the end of the calendar year.
- On the Advice Letter Filing Summary Form, enter GO 133-E Service Quality Fine Tabulation as the Advice Letter Subject.

Upon approval of the Tier 2 Advice Letter related to fines by the CPUC, carriers are required to remit the assessed fines as specified in the advice letter within 30 days. Fine payments should be made by check or money order, payable to the California Public Utilities Commission via mail or in person at the following address:
